Job Title: CAREER PLANNER

Location: Hawthorne

Job Description: Responsible to coordinate, develop, create and implement employment opportunities using Person Centered Planning methods and techniques in accordance with the needs of the individuals served. Other responsibilities include building relationships with families, employers and regulatory entities to serve and support the individuals served. Requires HS Diploma/GED, a valid NYS Driver’s License, a reliable car and one (1) year experience with DD population; Bachelor’s degree in related field is preferred. Must be flexible, reliable, and take initiative. Ability to work effectively with employers, families, interdepartmental staff and other community and corporate contacts.

 

Job Title: COMMUNITY CHOICES INSTRUCTOR

Location: Hawthorne 

Job Description: Function as a consumer instructor.  Develop and implement community based consumer programming/ integration in accordance with their needs, preferences and desires as per their Individual Service 
and Habilitation Plan.  Maintain health and safety standards. Position will specifically work within the “Choices” Without Walls program. Staff may be required to operate wheelchair accessible vehicle.  Requires H.S Diploma, NYS Driver’s License, successful completion of OPWDD approved CPR/first aid certification.

 

Job Title: CLINICAL TEAM LEADER

Location: Hawthorne

Job Description: For assigned residences, oversee and supervise the delivery of residential services to individuals and ensures compliance to all pertinent regulations. This includes ensuring the provision of clinical and healthcare services to meet individuals’ needs as prescribed in the individual service plans and that opportunities for learning and choice are provided to residents ongoing. Requires a Master’s Degree in Psychology, Social Work, or Special Education, and must be a QIDP with at least one-year experience working with persons with developmental disabilities. Ability to effectively supervise and train residential staff, great communication skills.

 

Job Title: COMMUNITY HABILITATION COUNSELOR PER DIEM

Location: Westchester County

Job Description: Work with a person with developmental disability in their home.  Provide individual with assistance in acquiring and maintaining life skills that enable the individual to be independent. Activities are based on valued outcomes in accordance with the individual’s service plan. Requires HS Diploma and a valid NYS Driver’s License.

 

Job Title: COMMUNITY HABILITATION SUPERVISOR

Location:  Hawthorne        

Job Description: Provide direct Community Habilitation services as well as supervise Community Habilitation staff. The Community Habilitation Supervisor will attend annual and semi-annual meetings, act as primary contact to families and fill-in for Community Habilitation staff as needed.  The supervisor will write Community Habilitation plans and track billing in Medisked to ensure proper billing standards are followed. This person will also provide training to staff for both annual and periodic in-service training. Requires Bachelor’s Degree in a Human Services or related field and at least 1-year experience; or a HS Diploma with at least 3 years’ experience working with individuals with developmental disabilities. A valid NYS driver’s license is required. Supervisory experience preferred.

 

Job Title:  COORDINATOR OF COMMUNITY CHOICES

Location: Hawthorne                               

Job Description: Responsible for the development and delivery of community based Day Habilitation, Supported Employment, and Blended Services to the individuals under their supervision in accordance with their ISP. Assist with development, organization and implementation of consumer programming. Overall supervision of assigned staff including ensuring all required documentation is accurate and completed in a timely fashion. Assure maintenance of health and safety standards within facility including adequate staff coverage and has the responsibility of transporting individuals to and from program on an ongoing substitute basis. Staff will be required to provide program coverage and operate wheelchair accessible vehicle. Assume the QMRP role as assigned and required. Requires Associate’s degree in related field, Bachelor’s degree preferred; a valid NYS Driver’s License and one-year experience working with DD population, individuals with disabilities or related field. Basic computer skills required; supervisory experience preferred. 

 

Job Title: JOB PLACEMENT SPECIALIST

Location: Hawthorne, Yonkers and Mount Kisco

Job Description: Act as a liaison and advocate for individuals with disabilities employed in the community. Develops a Habilitation Plan and is responsible to implement the valued outcomes of the plan. Maintain contact with employers, families and service coordinators regarding progress or issues at the work site. Services may include job skills, interpersonal skills, job advancement skills and job retention skills.  Requires a NYS valid Driver’s License and a H.S. diploma/GED. AA degree in Human Services or related field or Bachelor’s degree is preferred. One year experience with DD population, individuals with disabilities or related field is preferred.

 

Job Title: LICENSED CLINICAL SOCIAL WORKER  (LCSW)  Bilingual Spanish

Job Description: Arc of Westchester Article 16 Clinic seeks a NYS Licensed Clinical Social Worker, LCSW on a contracted basis.  The position will conduct psychotherapy to individuals with intellectual and developmental disabilities. The successful candidate must hold and maintain a current NYS Clinical license. Must have knowledge and experience in the identification and treatment of mental health. Experience with individuals with developmental and intellectual disabilities is preferred. Candidate must be Bilingual Spanish.

Job Description: The RN provides nursing services to individuals of the residences and programs assigned, assists in the training of other Registered Nurses and department staff. Acts as a resource to Nursing Supervisor and administration regarding health care issues.  Requires Graduation from an accredited college or university nursing program, Bachelor’s degree preferred; current NYS license as a Professional Registered Nurse; experience working as a nurse in OPWDD approved program preferred. Job Description: Provide families of children and adults with mild to severe developmental disabilities with respite services on weekdays and weekends. Attend to physical, safety, social and recreational needs of consumers staying at the house. Assist and instruct guest in the areas of self care, independent living, communication and socialization and physical health. Create a clean environment for guest to live in. Job Description: The Children's School has several collaborative inclusion classes located in community early childhood settings for preschool students with a disability.  We are seeking a certified special education teacher, experienced working with young children with developmental delays and disabilities.  In addition to classroom management, curriculum development and lesson planning to design activities that meet the IEP goals for students, this position requires communication with parents, school districts and other professional clinicians, working on a collaborative team. Requires Graduation from College or University with a Masters Degree in Special Education and at least two years of experience.
